What Causes Tooth Decay?

Decay results from repeated acid attacks that dissolve minerals from enamel faster than saliva can replace them. Frequency matters more than quantity.

The mechanism in plain terms

The mouth hosts a complex community of bacteria. Some of them metabolise fermentable carbohydrates — sugars and refined starches — and produce acid as a by-product. That acid lowers the pH at the tooth surface and pulls calcium and phosphate out of the enamel.

Saliva then does the opposite work: it dilutes and neutralises acid, and redeposits minerals. Decay develops when the demineralising side of that exchange consistently outpaces the remineralising side.

Frequency beats quantity

Each exposure to fermentable carbohydrate triggers an acid drop that takes time to recover from. A dessert eaten in ten minutes produces roughly one such episode. The same amount of sugar sipped across an afternoon produces many.

This is why advice focuses on how often you eat and drink rather than only on how much sugar is involved, and why frequent sweetened coffee, sports drinks or hard candies are singled out.

Why some people get more cavities

Risk is not distributed evenly, and much of the difference is not about effort.

  • Reduced saliva flow from medication, medical conditions or dehydration
  • Deep grooves in molars that hold plaque beyond the reach of bristles
  • Crowded or overlapping teeth that are difficult to clean between
  • Exposed root surfaces after gum recession, which soften at a higher pH than enamel
  • Existing restorations with margins that trap plaque
  • Fluoride exposure through water, toothpaste and professional application

What actually slows the process

Fluoride toothpaste used twice daily is the single best-supported home measure, because fluoride makes enamel more resistant and encourages remineralisation. Cleaning between the teeth addresses the surfaces where decay most often starts. Reducing the number of sugar exposures per day changes the arithmetic directly.

Early demineralisation — the chalky white spot stage — can sometimes be arrested without drilling. Once a cavity has broken through into dentine, restoration is usually required.
